Welcome. The 3D Healing Apothecary is a live, online space where you can ask general questions about nutrition and dietary supplements. Please read the following before entering. By checking "I agree" and typing your name, you confirm that you understand and accept these terms.
1. Educational purpose only. Visits are for general educational and informational purposes. Dr. James Macie, D.C., will share general information about nutrition, supplements, and product labels. He will not examine, diagnose, or treat any disease or condition during an Apothecary visit, and nothing discussed is a substitute for care from your own physician or licensed healthcare provider.
2. No provider-patient relationship. A drop-in visit does not create a chiropractor-patient, physician-patient, or other healthcare provider-patient relationship. Dr. Macie is licensed as a chiropractor in Texas. If you are located outside Texas, information shared is general education only.
3. Talk with your own provider. Always consult your physician or qualified healthcare provider before starting, stopping, or changing any supplement, diet, or program, especially if you are pregnant or nursing, take prescription medication, have a medical condition, or are scheduled for surgery.
4. FDA statement. Statements made during visits and about products have not been evaluated by the Food and Drug Administration. Products discussed are not intended to diagnose, treat, cure, or prevent any disease. Individual results vary, and no specific outcome is promised.
5. Not for emergencies. The Apothecary is not an emergency or urgent-care service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
6. Age requirement. You must be 18 years of age or older to enter.
7. Group setting and privacy. Some sessions are open to multiple visitors at once, and others present may see and hear what you share. Please do not share personal health details you would not want others to hear; if you would like a private conversation, you may ask to speak one-on-one or book a Complete Nutritional Assessment. Information you submit on this form is used to manage visits and is handled as described in our Privacy Policy. We do not sell your personal information.
8. Recording. Apothecary sessions are not recorded unless all participants are notified and agree in advance. You agree not to record, photograph, or broadcast the session or other visitors without their permission.
9. Products and financial disclosure. Dr. Macie may recommend products, including through his online dispensary. He may earn income on products purchased through his dispensary or links he shares. You are never obligated to purchase anything, and you are free to buy any product wherever you choose.
10. Conduct. Please be respectful of Dr. Macie and other visitors. Visitors who are disruptive, abusive, or who share inappropriate content may be removed without notice.
11. Voluntary participation. Your visit is voluntary. You may leave at any time, and Dr. Macie may end a visit or decline to discuss any topic at his discretion.
12. Technology. Visits take place through a third-party video platform. Connection quality, availability, and the platform's own privacy practices are outside our control.
13. Acknowledgment. By checking "I agree" and typing my name below, I confirm that I am 18 or older, that I have read and understand this Consent & Acknowledgment, and that I voluntarily agree to its terms. I understand my electronic signature has the same effect as a handwritten signature. This agreement is governed by the laws of the State of Texas.
